Em estudos medicos, ha melhor que isso, e moleculas mais toleraveis.
O amisulpride por exemplo, ja aqui mencionado.
È o antipsicotico de segunda geracao mais eficaz, e com afinidade por um receptor serotonogenico, o 5ht7.
Alias este amisulpride è das moleculas mais misteriosas, porque tem duas acoes distintas, em baixa dose e em alta dose. O psiquiatra de uma amiga minha, meteu-a logo em low dose amisulpride, porque actua logo, ao fim de dois dias estava sem depressao. Um prozac leva 1-2 meses a actuar. Em termos de preco, penso que o amisulpride è barato, nao posso garantir com a certeza mas è cerca de 3 euros comparticipado.
Esta molecula è ou esta a ser a mais prescrita na europa a ultrapassar as outras. E nao ha quem se sinta mal com ela. Em caso de duvidas, è pesquisar sobre o amisulrpide, os estudos sao conclusivos, è uma molecula milagre.

Eu nao sei o problema do vanilla. Apenas emiti o que sei e o que encontrei sobre outras moleculas.
Vamilla tu fumas? 30% dos fumadores tem algum tipo de patologia mental. A hipoxia e falta de antioxidantes nao perdoam. Os niveis de SOD em fumadores è super baixo e o tabaquito vai ao brain em 10 segundos.
Uma outra toxicidade qualquer tb pode esquzofrenar uma pessoa. È preciso ver onde uma pessoa vive e como vive, onde dorme, o que come. E o perfil de nutrientes. Os esquizo por norma tem falta de cobre selenio e magnesio e uma outra que nao me lembro.
Mas molecula por molecula, eu iria para a referida e largaria o Invega

Nao é preciso , eu ate acho que os sapatos foram dados..... os acabamentos estao ao nivel de sapatos caros , sao super confortaveis, vamos ver quanto tempo é que vao durar.
Por 39,95 euros foram dados....valem algumas vezes mais

WHAT IS SCHIZOPHRENIA?
Schizophrenia is a loaded word, feared by patient and public alike. It conjures up images of dangerous and crazy people. In truth, most members of the public have no real idea what is meant by this word, often believing that sufferers have split personalities, like Jekyll and Hyde. About one in a hundred people have schizophrenia and can have a variety of symptoms which are known as ‘positive’ symptoms and ‘negative’ symptoms.
WHAT CAUSES SCHIZOPHRENIA?
As is the case with most mental illness, the cause of schizophrenia is not known. There are a number of theories relating to neurotransmitter imbalances and functional magnetic resonance imaging (fMRI) studies have shown a broad array of brain abnormalities.
DIET AND NUTRITION...WHAT WORKS
Balance your blood sugar and avoid stimulants
Your intake of sugar, refined carbohydrates, caffeine, alcohol and cigarettes, as well as stimulant drugs, all affect the ability to keep one’s blood sugar level balanced. On top of this common antipsychotic medication may also further disturb blood sugar control. Stimulant drugs, from amphetamines to cocaine, can induce schizophrenia. The incidence of blood sugar problems and diabetes is also much higher in those with schizophrenia.
Increase essential fats
We build our brain from specialised essential fats. Of course, this isn’t a static process. We are always building membranes, then breaking them down, and building new ones. The breaking down, or stripping of essential fats from brain, membranes, is done by an enzyme called phospholipase A2 (PLA2). This is often overactive in people with schizophrenia, and this leads to a greater need for these fats, which are quickly lost from the brain. This explains earlier findings that schizophrenic patients have much lower levels of fatty acids in the frontal cortex of the brain. So, what’s the evidence that increasing a person’s intakes of essential fats makes a difference?
Up antioxidants
There’s another part to the essential fat story. These fats are also prone to destruction in the brain, and in the diet, by oxidants. Indeed, there is evidence of more oxidation in the frontal cortex of those with schizophrenia. Therefore, as well as increasing the intake of essential fats, it makes sense to follow a diet (and lifestyle) that minimises oxidants from fried or burnt food and maximises intake of antioxidant nutrients such as vitamins A, C and E. These alone have been shown to help. Vitamin C is also an anti-stress vitamin and may counter too much adrenalin, which is often found in those diagnosed with schizophrenia. Smoking is both a source of oxidants and destroys vitamin C.
Consider niacin
One of the classic vitamin deficiency diseases is pellagra – Niacin (vitamin B3) deficiency. The classic symptoms of this condition are the '3 Ds' – dermatitis, diarrhoea and dementia. A more extensive list of symptoms might include headaches, sleep disturbance, hallucinations, thought disorder, anxiety and depression.
Methylation, B12, folic acid and B6
Methylation is a critical process in the brain that helps maintain the right chemical balance. An indicator of faulty methylation is having a high level of a toxic amino acid in the blood called homocysteine. The body makes homocysteine from dietary protein and, provided you are getting enough of certain vitamins1, especially folic acid, B12 and B6, homocysteine levels decrease. Many people with schizophrenia, especially young males, tend to have a high level of homocysteine, despite no obvious dietary lack of these vitamins. High levels of homocysteine and low blood levels of folic acid have been reported by many research groups. These unusually high levels don’t appear to relate to diet or lifestyle factors, such as smoking. People diagnosed with schizophrenia are more likely to have inherited a genetic variation of a key homocysteine lowering enzyme, which may make them need more of these and other nutrients.
Are you pyroluric? The zinc link
Possibly one of the most significant ‘undiscovered’ discoveries in the nutritional treatment of mental illness is that many mentally ill people are deficient in vitamin B6 and zinc. But this deficiency is no ordinary deficiency: you can't correct it by simply eating more foods that are rich in zinc and B6. It is connected with the abnormal production of a group of chemicals called ‘pyrroles’. A person with a high level of pyrroles in the urine needs more B6 and zinc than usual, since they rob the body of these essential nutrients, increasing a person’s requirements to stay healthy. At the Brain Bio Centre, we find many people diagnosed with schizophrenia have ‘pyroluria’.
Check for allergy
Some people with mental health problems are sensitive to gluten, especially wheat gluten, which can bring on all sorts of symptoms of mental illness. This has been known since the 1950s, when Dr Lauretta Bender noted that schizophrenic children frequently had coeliac disease (severe gluten allergy). By 1966 she had recorded 20 such cases from among around 2,000 schizophrenic children. In 1961 Drs Graff and Handford published data showing that four out of 37 adult male schizophrenics admitted to the University of Pennsylvania Hospital in Philadelphia had a history of coeliac disease in childhood.

Isto è mais mera curiosidade em como o tabaco mexe com o figado violentamente.
Nao è muito invulgar um fumador precisar de dosagem dupla em certas medicacoes, anti inflamatorios, sedativos, etc

Cigarette smoking remains highly prevalent in most countries. It can affect drug therapy by both pharmacokinetic and pharmacodynamic mechanisms. Enzymes induced by tobacco smoking may also increase the risk of cancer by enhancing the metabolic activation of carcinogens. Polycyclic aromatic hydrocarbons in tobacco smoke are believed to be responsible for the induction of cytochrome P450 (CYP) 1A1, CYP1A2 and possibly CYP2E1, CYP1A1 is primarily an extrahepatic enzyme found in lung and placenta. There are genetic polymorphisms in the inducibility of CYP1A1, with some evidence that high inducibility is more common in patients with lung cancer. CYP1A2 is a hepatic enzyme responsible for the metabolism of a number of drugs and activation of some procarcinogens. Caffeine demethylation, using blood clearance or urine metabolite data, has been used as an in vivo marker of CYP1A2 activity, clearly demonstrating an effect of cigarette smoking, CYP2E1 metabolises a number of drugs as well as activating some carcinogens. Our laboratory has found in an intraindividual study that cigarette smoking significantly enhances CYP2E1 activity as measured by the clearance of chlorzoxazone. In animal studies, nicotine induces the activity of several enzymes, including CYP2E1, CYP2A1/2A2 and CYP2B1/2B2, in the brain, but whether this effect is clinically significant is unknown. Similarly, although inhibitory effects of the smoke constituents carbon monoxide and cadmium on CYP enzymes have been observed in vitro and in animal studies, the relevance of this inhibition to humans has not yet been established. The mechanism involved in most interactions between cigarette smoking and drugs involves the induction of metabolism. Drugs for which induced metabolism because of cigarette smoking may have clinical consequence include theophylline, caffeine, tacrine, imipramine, haloperidol, pentazocine, propranolol, flecainide and estradiol. Cigarette smoking results in faster clearance of heparin, possibly related to smoking-related activation of thrombosis with enhanced heparin binding to antithrombin III. Cutaneous vasoconstriction by nicotine may slow the rate of insulin absorption after subcutaneous administration. Pharmacodynamic interactions have also been described. Cigarette smoking is associated with a lesser magnitude of blood pressure and heart rate lowering during treatment with beta-blockers, less sedation from benzodiazepines and less analgesia from some opioids, most likely reflecting the effects of the stimulant actions of nicotine. The impact of cigarette smoking needs to be considered in planning and assessing responses to drug therapy. Cigarette smoking should be specifically studied in clinical trials of new drugs.

Introduction

Despite anti-smoking campaigns, there are about 1.3 billion cigarette smokers worldwide and this number is still increasing.1 Both smoking prevalence and daily tobacco consumption are very high in patients with psychiatric disorders.2 Over two-thirds of Australians with psychosis smoke cigarettes, compared with one quarter of the general population.3

Smoking restriction or cessation is now commonly imposed on patients by 'no smoking' policies in Australian hospitals. Stopping smoking is particularly important in patients with mental health problems as they often have an adverse cardiovascular risk profile (from factors including obesity, dyslipidaemia, insulin resistance and hypertension). This is associated with high rates of premature mortality from cardiovascular disease.4

Even with nicotine replacement therapy as a cessation aid, patients with psychiatric illness appear to have more difficulty maintaining long-term abstinence than other smokers.5 Most resume smoking within five weeks of hospital discharge.6 These patients should be regularly asked about their smoking status, as an additional risk of resuming or abruptly ceasing cigarette smoking has the potential for drug interactions.7

Drug interactions

The chemicals in smoke may interact with antipsychotics, antidepressants, benzodiazepines,8 oral contraceptives, inhaled corticosteroids and beta blockers via pharmacokinetic and pharmacodynamic (often nicotine-mediated) mechanisms.9

Pharmacokinetic interactions

Cigarette smoking induces the activity of cytochrome P450 (CYP) 1A2 (via chemicals in cigarette smoke such as polycyclic aromatic hydrocarbons)10 and also CYP2B6.11 These enzymes metabolise several clinically important drugs (such as antidepressants and antipsychotics) (Box) and a number of procarcinogens (such as those in cigarettes).10,12

The effect of smoking on hepatic enzymes is not related to the nicotine component of tobacco. Nicotine replacement therapy does not influence CYP1A2 activity.13

Genetic polymorphisms of the CYP1A2 gene contribute to extensive inter-individual variability in drug metabolism14,15 and are associated with altered inducibility of gene expression in smokers.7,16 There are also marked ethnic differences in the distribution of CYP1A2 mutations,10,17 meaning that different ethnic groups respond differently when the patient stops smoking.

CYP1A2 activity is significantly higher in heavy smokers (more than 20 cigarettes/day) than in nonsmokers.18 This is likely to be clinically relevant for some drugs which have a narrow therapeutic index and are metabolised by CYP1A2, such as clozapine. The induction varies depending on the bioavailability of the components of cigarette smoke and the extent of inhalation.8 It is not known how the number of cigarettes smoked daily or inter-individual variation affects CYP1A2 induction,9 but heavier smokers appear to have a greater increase in the clearance of drugs.19

This enzyme induction is rapidly reversed when patients abruptly stop smoking, with a new steady state of CYP1A2 activity reached after approximately one week.20 This reduction in enzyme activity reduces clearance and increases the risk of adverse drug reactions for patients taking drugs metabolised by CYP1A2.20,21 These patients should be regularly asked about their smoking and the extent of their cigarette consumption.16

Clozapine and olanzapine
Cigarette smoking induces the metabolism of clozapine and olanzapine,22 resulting in lower plasma concentrations.7,16,21 The daily consumption of 7–12 cigarettes is probably sufficient to cause the maximum induction of clozapine and olanzapine metabolism.22 A 50% difference in the mean daily dose of clozapine needed by smokers and nonsmokers to reach a given blood concentration has been reported.23

Irrespective of smoking status, the mean oral bioavailability of clozapine is 27–47% and clozapine plasma concentrations have more than a 45-fold variability amongst individuals during chronic treatment.24 There are also large inter-patient differences in olanzapine exposure, with gender and genetic factors contributing.25

Non-smokers are at higher risk of adverse effects if treated with standard doses, suggesting that there is an interaction between smoking, olanzapine and clozapine.20,22 In one case report, a patient receiving olanzapine experienced extrapyramidal symptoms (including akathisia, akinesia and bradyphrenia) within days of significantly reducing tobacco consumption.21 Case reports on smoking discontinuation by patients taking clozapine outline effects including confusion,21 tonic–clonic seizures, stupor, coma26 or aspiration pneumonia.27

Clearance of clozapine has been shown to decrease when smoking is ceased, with a mean increase of 72% in plasma clozapine concentrations.27 It is suggested that daily dose reductions (of approximately 10% until the fourth day after smoking cessation) should be made whenever patients cease smoking during treatment with clozapine.20 Patients who resume smoking after leaving hospital may need their drugs and doses reviewed to account for this change.28 Therapeutic drug monitoring of clozapine is useful.

Antidepressants
As fluvoxamine is metabolised by CYP1A2, smokers might require higher doses than those recommended from clinical trial data.8 Smoking is not anticipated to alter the pharmacokinetics of other selective serotonin reuptake inhibitors as they are not substrates of CYP450 isoenzymes induced by smoking.8

Smokers might require higher than normal doses of the tricyclic antidepressant imipramine. They do not appear to require dose adjustments of amitriptyline, nortriptyline or clomipramine.8

Warfarin
Warfarin's less active R isomer is eliminated to a minor extent by CYP1A2.29 Smoking may therefore potentially interact with warfarin by increasing its clearance and reducing its effect. A recent meta-analysis showed that smoking appeared to increase the warfarin dose requirement by 12%, resulting in an extra 2.26 mg per week compared with nonsmoking.30 Consequently, INR should be closely monitored when there is a change in patients' smoking status.

Clopidogrel and prasugrel
CYP isoenzymes (including CYP2C19, 3A4/5, 1A2, 2B6 and 2C9) convert clopidogrel and prasugrel into their active metabolites, which bind irreversibly to the receptors on platelets. As smoking is known to enhance CYP1A2 activity, theoretically it could increase the antiplatelet efficacy of these thienopyridine drugs.31

An enhanced response to clopidogrel has been seen in smokers who are CYP1A2 (163CA) A-allele carriers.32 Two retrospective analyses of large randomised clinical trials of clopidogrel showed that clopidogrel might be more effective in active smokers.33,34 However, a systematic review concluded that smoking is not associated with reduced platelet reactivity in patients on clopidogrel.31 Genetic polymorphisms seem not to impact on the activity of prasugrel.31

Caffeine
Caffeine is highly dependent on CYP1A2 for its metabolism. Smokers require up to four times as much caffeine as non-smokers to achieve the same plasma caffeine concentration. Caffeine can increase the concentration of clozapine and olanzapine.35

Pharmacodynamic drug interactions

Pharmacodynamic drug interactions with tobacco smoke are largely due to nicotine.9

Methadone
The vast majority of patients using methadone maintenance therapy also smoke tobacco.36 Methadone doses have been found to be higher in heavy smokers37 and methadone has been shown to increase both smoking rates and smoking satisfaction.38 Patients report less methadone-induced sedation when they smoke around the time of their methadone dose.39

Although methadone is a CYP2B6 substrate (Box), nicotine affects the endogenous opioid system. Cigarette smoking enhances the effect of methadone on opioid withdrawal symptoms.40

Methadone attenuates nicotine withdrawal. Reducing methadone doses when the patient is trying to stop smoking could be detrimental.40



Benzodiazepines
Nicotine activates the central nervous system9 and this may explain the attenuated sedation observed in smokers compared to non-smokers taking benzodiazepines.41 Prescribers should be aware that when patients taking benzodiazepines stop smoking, there is a risk of central nervous system depression.

Oral contraception
Smoking increases the adverse effects of the combined oral contraceptive pill (specifically thromboembolism, ischaemic stroke and myocardial infarction). The combined oral contraceptive pill is contraindicated in women aged 35 years or older who smoke 15 or more cigarettes a day.9 For smokers who use combined low-dose oral contraceptives, the attributable risk of death from cardiovascular disease is 19.4 per 100 000 women aged 35–44 years (vs 3.03 per 100 000 for non-smoking women of the same age).42 This risk is also presumed to be associated with other contraceptives containing oestrogen.9

Limited data suggest no convincing association between cardiovascular disease and progestogen-only pill use.43 If smoking cessation is unsuccessful, non-hormonal or progestogen-only contraceptives are preferred from a cardiovascular perspective.9

Other drugs
The efficacy of inhaled corticosteroids may be reduced in asthmatic patients who smoke,9 so these patients might require higher doses of inhaled corticosteroids to attain asthma control.44

Proposed mechanisms of corticosteroid insensitivity include suppression of histone deacetylase expression and activity by cigarette smoking, causing inflammatory gene expression and a reduction in glucocorticoid function.45 Clearance of corticosteroids from the lungs may be altered by increased mucus secretion or airway permeability.46

Smokers may require higher doses of beta blockers. Although propranolol is a CYP1A2 substrate (Box), nicotine-mediated central nervous system activation may diminish the effect of beta blockers on blood pressure and heart rate.9

Nicotine, psychosis and dopamine
The researchers do, however, propose another hypothesis that could explain the association. Sir Robin Murray, professor of Psychiatric Research at the IoPPN, points to the brain's dopamine system:

"Excess dopamine is the best biological explanation we have for psychotic illnesses such as schizophrenia. It is possible that nicotine exposure, by increasing the release of dopamine, causes psychosis to develop."

"Longer-term studies are required to investigate the relationship between daily smoking, sporadic smoking, nicotine dependence and the development of psychotic disorders," says IoPPN research fellow Dr. Sameer Jauhar.

"In view of the clear benefits of smoking cessation programs in this population every effort should be made to implement change in smoking habits in this group of patients."

A 2014 study conducted by researchers from the Washington University School of Medicine in St. Louis, MO, found that people with severe mental illness such as schizophrenia or bipolar disorder have a higher risk for substance abuse - especially cigarette smoking.

In that study, people with severe mental illness were:

4 times more likely to be heavy alcohol users (four or more drinks per day)
3.5 times more likely to use marijuana regularly (21 times per year)
4.6 times more likely to use other drugs at least 10 times in their lives
5.1 times more likely to be daily smokers.

Fogo uma coisa boa da reforma por invalidez é que mantenho á quatro anos a mesma medicação com consultas regulares ao psiquiatra, quando dava aulas comecei com o risperdal depois o meu médico tirou e comecei com a olapanzina comecei com 5 mg e já ia em 15 mg e tive que introduzir o invega 6 mg. mas desde que me reformei mantive sempre a mesma medicação,
